  • We do NOT accept ANY form of Medicare.
     

  • Copay's are due at time of service. Credit/Debit/HSA Accepted.
     

  • If we are out of network for your plan, you are welcome to pay out of pocket. We can provide a detailed superbill so you can submit it to your insurance for possible reimbursement, which varies by plan.

  • Medications will not be adjusted between appointments. Please contact the office to schedule a visit for any medication changes.
